Wendy was born in Columbus, Ohio to James Sladek and Ann D. Carver on May 20, 1950.

Wendy leaves behind her loving and adoring aunt Margaret Pittius as well as many cousins.

Wendy is predeceased by her parents, James and Ann Sladek.
